Commonly evaluated factors
Who is typically considered
- Adults with BMI ≥ 35 (class II–III obesity)
- Adults with BMI 30–34.9 and significant metabolic disease (e.g., type 2 diabetes, hypertension, dyslipidemia)
- Patients who have not achieved durable results with medical and lifestyle therapy
- Patients able to commit to long-term follow-up, nutrition, and supplementation
- Patients with realistic expectations about outcomes and risks
- Patients without active medical or psychiatric conditions that would unreasonably increase surgical risk
Generally not considered candidates
Situations that require further evaluation or alternative care
- Untreated severe psychiatric illness or active substance use disorder
- Untreated active eating disorders
- Pregnancy
- Uncontrolled medical conditions that significantly elevate surgical risk
- Inability or unwillingness to commit to lifelong follow-up and lifestyle change
Evaluation process
What a complete workup typically includes
Medical history & exam
Comprehensive review of weight history, comorbidities, medications, prior surgeries, and overall health.
Laboratory & imaging
Labs for metabolic, nutritional, and endocrine status; additional studies as clinically indicated.
Nutritional assessment
Dietitian review of intake patterns, supplementation needs, and pre-operative dietary preparation.
Behavioral assessment
Screening for eating disorders, mood disorders, and readiness for sustained lifestyle change.
Risk-benefit discussion
Detailed review of procedure options, risks, alternatives, and expected outcomes.
Shared decision-making
Final candidacy decision is collaborative between the patient and the multidisciplinary team.
